More about our Added Benefits Plan
Our Added Benefits fee is a modest per-child administrative fee that includes services that may be non-covered or non-reimbursed by your insurance company. This fee allows us to give your family exceptional one-on-one care.
Our Added Benefits fee covers the following:
- Extended Well Visit Times & guaranteed same day sick visits
- Forms completed within 1 business days (School, Camp, Sports, and others)
- Return to school/daycare and PE excuse notes within 24 hours
- Letters and reports
- Access to the patient portal for messaging with providers, prescription refills, appointment requests & more!
- 24 Hour Access to the on-call provider for after hours emergencies
This fee is based on the age your child will be as of 1/1/2026.
This payment is to be submitted via the payment page (link below) or by calling the office. We will not automatically charge this fee to the card on file.
- Newborns up to 1 year: $200 per infant
- Children 1 year & older: $250 per child
- Young Adults 18+: $175 per child
- Family Maximum: $750.
- No Fee for Husky/Medicaid Patients
Early Bird Discount $20 PER FAMILY if paid between Monday, 12/1/25 through Monday, 1/5/26. (please manually deduct $20 from your total, the system will not automatically deduct the discount)
Payment Due no later than Sunday, 2/1/26
$50 Late Fee automatically applied to any payments received after Monday, 2/2/26.
(If we have not received your payment by 2/1/26, the card on file will be auto charged with the late fee applied)

As HSA and FSA Programs vary greatly, please check with your program administrator to determine if this Added Benefits Fee is HSA or FSA eligible.
